Please arrive for your scheduled
appointment about 15 minutes early. You will have forms to fill out. The necessary information will include:
- personal information about the patient and parents
- medical history of the patient
- insurance information for the patient
- any records that you might have from other orthodontists or dentists
Dr. Gaudry will examine the patient and talk with you about what he sees.
If he sees an orthodontic problem, he will suggest a treatment plan. Ramie will go into more detail with you and show pictures
of the treatment plan/appliances, and more.
Ramie and Jenny will also work with you to find what
type of insurance or other program coverages you have, and suggest various methods of payment. That way, you can leave the
office with a variety of solutions to your problem.
If you wish, we can take records of the patient at this
visit. These include but are not limited to x-rays and impressions.
From this information, Dr.
Gaudry will write a treatment plan for the patient, and will sit down with you for a consultation. At this consultation, he
will describe in detail how he will correct the problem, how long treatment should last, what you can expect from him, what
type of cooperation he expects from the patient, and what it will cost you.
We feel that it is most important that you and the patient understand the treatment
and what is expected. AT ANY TIME, please feel free to ask questions. We want you to be involved in the process. Our success
